Free Orthopedic Sports Injury Evaluations September 8, 2012

Arizona Orthopedic Surgical Hospital (AOSH) is offering free Orthopedic Sports Injury Evaluations on Saturdays this fall. These sessions will be conducted by orthopedic surgeons who specialize in sports medicine and include both an evaluation and treatment recommendation.
Athletes should come prepared with the following:

Name
Address
Date of birth
Social Security Number
Home phone number
Parent(s) name, employer and work phone number
Name of school
Name of athletic trainer/coach
Proof of insurance (AOSH will make arrangements for athletes without insurance)
Copy of films for any x-rays or MRI taken prior to this visit

Diagnostic testing, such as an x-ray or MRI, is not included as part of the evaluation. Athletes will be responsible for insurance co-pays, deductibles and services not covered by insurance. A parent, guardian, athletic trainer or coach must be present.
Evaluations will be available on the following Saturdays. Registration begins at 6:30 a.m., with examinations from 7 to 9 a.m.

Sept. 8, 15, 22 and 29
Oct. 6, 13, 20 and 27
Nov. 3, 10 and 17
